What to do if I was sent to the emergency room for emergency surgery but the surgery was not called in by the hospital?

Question Details: It claims to not have any of my insurance information. This was not my first time as a patient in this particular hospital. I was not informed that the bill was not being covered by my insurance until recently. I was told that the hospital was the ones responsible for making the call for approval by the insurance company because it was emergency surgery. The hospital refuses to pay the bill because they claim they had none of my insurance information. I find this hard to believe because I am definitely in the system. When I was being admitted I was not asked about insurance I was told I was all registered.
