Can I file a subpoena for documents even if I have not filed for civil action in federal or state court?

An individual cannot issue a subpoena. You will need an attorney or a judge to issue a subpoena. It would be advisable to be represented by an attorney, who can issue a subpoena for the documents and medical records you need.
