We have a house that was transferred from my grandparents years ago. The deed reads as dad’s name, a married individual, my name a single individual, brother’s name, a single individual, as joint tenants with right of survivorship and not tenants in common. herein called

So long as title was held as "jont tenants with right of survivorship", then upon the death of 1 owner, the other 2 owners became vested with that deceased owner share. In other words, you and your mother each now own 1/2 of the property.

A joint tenancy with right of survivorship means upon the death of a joint tenant, the surviving joint tenant inherits the property. 
You and your mom are the surviving joint tenants and owners of the home, which both of you have inherited.
